IMPORTANT INFORMATION and RISK DISCLOSURE Risk Disclosure Forms - Each park in New Zealand has risk disclosure and acknowledgement forms for all participants that use the parks. This requires that every person: Over 16 Years - need to read and sign the Risk Disclosure form before taking part in Adrenalin Forest. Under 16’s, a parent or guardian needs to sign the Risk Disclosure form for them as stating that they are responsible for them and they are capable of undertaking the activities at Adrenalin Forest. The ratio is 1 parent/guardian to 12 children (under 16). So if you have 13 children under 16 you will need 2 parents/guardians for them to participate in Adrenalin Forest and so on. Supervision of Under 16’s – As stated on the Risk Disclosure Form for Under 16’s – the parent/guardian (which can be a teacher) must actively supervise those under their responsibility while they are in the park. The parent/guardian will need to attend the safety briefing with those under their responsibility and then follow and supervise them while they move around the course. The parent/guardian may for-fill this roll on the ground or on the course. PLEASE NOTE: - Height restrictions are due to the positioning of the safety cables so there is no leniency on heights. - If you are over 1.40m tall then you pay for the full course, it is then up to you how far you go. - Please check the height of all members in your group to save disappointment on arrival at the park. - You have 3 hours to complete the entire course. - You can complete each course only one time. - Children U16 years must be actively supervised by a parent or guardian (over 18). One parent or guardian can only be responsible for 12 children under 16 years - Adrenalin Forest Staff supervise participants while on the course and are not responsible for them on the ground when they finish the courses. - We are an outdoor activity so please dress appropriately for the conditions on the day of your booking. We do not have any clothing to lend or hire. RISK DISCLOSURE - In accordance with the Adventure Activities Regulations 2016 we are required to disclose risks associated with our activities. - I understand that there is a risk of serious harm, death or injury associated with undertaking these activities. Injuries that can occur but are not limited to are: dislocations, fractures, lacerations, bumps, bruises, cuts, chipped teeth, sprains, fatigue. - I understand there are consequences if I ignore Adrenalin Forest’s safety management procedures and directions - I understand that there is a risk from natural events while undertaking these activities, events can include but are not limited to are: earthquakes, tsunamis, lighting, storms, high winds. - I agree that I will undertake the Adrenalin Forest activities in accordance with the Conditions of Entry and Safety Rules. - Before I climb and while on the pathways, I will attach my safety devices and remain attached to the safety line at all times. - I will be responsible at all times for wearing the safety equipment provided and fitted by the staff. I will not share any part of the safety equipment with anyone. I agree to use and treat the safety equipment as instructed in order to minimize the risk of user error or of malfunction due to incorrect use or treatment. - I consider that I am physically and mentally able to operate the safety equipment correctly and sustain the level of exertion and stress resulting from the Adrenalin Forest activities. To the best of my knowledge. I certify that I do not have any medical condition or special needs that may affect my own or others' safety. - I acknowledge that Adrenalin Forest will take all reasonable and practicable steps to keep me safe while I am involved in these activities. However, I agree that I also have a responsibility for my safety and accept full responsibility for my own actions or inactions. - Adrenalin Forest will refund you in full at the end of the safety briefing and demonstration if you are not willing or incapable of operating the safety equipment or of undertaking the risk.
